Health Needs by Grade
ALL students must have a medical/physical exam form on file according to the specifications noted below.

New to the District
At Registration, all forms will be available if needed.

  • Students new to the district have 30 days from the date of enrollment to submit all health documents.

New to Illinois
At Registration, all forms will be available if needed.

  • Child Health Examination
  • Immunization Records
  • Dental Exam
  • Vision Exam

Pre-Kindergarten
At Registration, all forms will be available if needed.

  • Child Health Examination
    • Lead Screening
  • Immunization Records
    • 4 doses of DTaP
    • 3 doses of IPV
    • 1 dose of MMR and Varicella
    • 4 doses of HIB
    • 3 doses of Hepatitis B
    • Pneumococcal series or one dose after age 2

Kindergarten
At Registration, all forms will be available if needed.

  • Child Health Examination
  • Immunization Records
    • 4th dose of IPV
    • 2nd dose of MMR and Varicella
    • 4 doses of DTaP
    • 4 doses of Polio
    • 3 doses of Hepatitis B
  • Diabetic screening
  • Dental Exam
  • Vision Exam

Grade 2
At Registration, all forms will be available, if needed.

  • Certificate of Child Health Examination Form on File
  • Immunization Records
    • Must have all shots listed for previous grades
  • Dental Exam

Grade 6
At Registration, all forms will be available, if needed.

  • Dental Exam
  • Child Health Examination
  • 1 dose of Tdap
  • 1 dose of the Meningococcal vaccine
  • Diabetic screening
  • Interscholastic sports marked YES if your child wants to do extra sports  

Grade 7 & 8
At Registration, all forms will be available, if needed.

  • Immunization Records
    • 2 doses of Meningococcal vaccine
  • Sports physicals must be renewed every calendar year. 


New physical means a physical done within the year of the start date of school on the new form.

The Clinic at Wal-Mart in Zion: 847-731-9617 no appointment necessary. Walk-ins welcome 7 days a week. (Public Aid, insurance, and cash are accepted.)

Vista Physician Group Office: 847-360-4260 for an appointment. (Public Aid, insurance, and cash are accepted.)

The Wal-Mart in Gurnee: 847-855-1242 with appointments, does eye exams only. (Public Aid, cash, and insurance are accepted.)

The Lind's Memorial Dental Clinic:
847-872-9227 by appointment only. (Public Aid, All Kids, and no income are welcome.)
