PreK Registration

Every student is required to submit a Birth Certificate (1), Proof of Residency (2), and Required Health Forms (3) during Registration. 

1. Birth Certificate
We do not accept hospital certificates. You may request a replacement Birth Certificate from Illinois Department of Health.

2. Proof of Residency
What are the ZESD 6 Proof of Residency requirements?

3. Health Forms
What are the ISBE and IDPH Health Requirements by grade?

4. Parent/Guardian ID

5. Proof of Household Income, most recent 30 days

6. Guardianship Custody Documentation, if applicable

7. School Transfer Forms, if applicable
ZESD 6 requires an ISBE Student Transfer Form for all new students transferring from a different public school in Illinois.  This form must be completed by the principal of the student's most recently attended school. 

 

How do I submit my student's required documents?

Drop off documents at the District Office, or bring them to your appointment with the District Registrar.

What to expect during  your appointment with the District Registrar?

• Families will complete an ASQ Parent Questionnaire and Home Language Survey for their child.
• All registering PreK students will be required to attend a developmental screening appointment, which can be scheduled during the initial registration appointment.

PreK Program admission is based upon highest need.

The Teaching & Learning Department will review your students application and eligibility.

If your student is accepted in to the PreK Program, a member of the Teaching & Learning Department will reach out via phone and email within 6 weeks.

If another language other than English is the main language spoken at home, an additional English Language Learners Screening appointment will be required if accepted into the program.

Snapcode
Once all documents have been approved and a student has been accepted to the PreK Program, parents will receive a Snapcode. This student-specific Snapcode is required to complete the Online Registration Application.

Online Registration Application
The Online Registration Application can be accessed through the PowerSchool MyPowerHub portal. During the registration process,  you will be provided your parent/guardian credentials to access the MyPowerHub portal.

The Online Registration Application link is located in the Forms section of MyPowerHub portal.

• Parents/guardians will have the option to select an English or Spanish registration form when entering their SnapCode information.

Using your student-specific Snapcode, complete the Online Registration Application.

Once the Online Registration Application is complete and all required documents are submitted, you will receive an email confirmation that your child is FULLY REGISTERED.

• If there is information or documentation that is incomplete, you will receive an email identifying registration is PENDING with information regarding outstanding details.
• You may return to the online registration portal to provide any further information or documentation required.

Students with FULLY REGISTERED status will be placed in class(es).
