PreK Registration

If you are interested in PreK, please complete the Interest Form. Submission of this form does not guarantee placement.

Pre-K Interest Form

• If the Prek Interest form is submitted before June 1, 2026, you will be contacted by June 1st regarding the next steps of the screening process.
• If the PreK Interest Form is submitted after June 1, 2026, you will be contacted within 10 days after submission.

• A screening invitation will be sent via email, including the following:
  • A Preschool Screening Questionnaire that collects information about your child and family.
  • A link to schedule your child's screening appointment.
• If appointment slots provided do not align with your schedule, please contact Lisa Traska at ltrasksa@zion6.org or 847-379-0126 to discuss alternative solutions.

You will be required to provide the required documents identified in Step 2.

• You must bring your child's birth certificate and proof of the most recent 30 days'  of income, which may include:
  • Proof of Benefits Letter
  • SNAP
  • WIC
  • Public Aid Documentation
  • Medicaid
  • Employment Paystubs
• Each child will be screened using the DIAL-4 Assessment.
  • The DIAL is an early childhood screening tool used to check important developmental skills. It includes simple activities and questions that help educators get a broad look at the child's motor, language, concept, self-help, and social development.
• The results from the screening will be shared with the parent/guardian.

• Within 10 days following the screening, the parent/guardian will be notified if the child has been accepted into the program.
• Upon program acceptance, the parent/guardian will be provided details to schedule a registration appointment.

Every student is required to submit a Birth Certificate (1), Proof of Residency (2), and Required Health Forms (3) during Registration.

5. Guardianship Custody Documentation, if applicable

ZESD 6 requires an ISBE Student Transfer Form for all new students transferring from a different public school in Illinois.  This form must be completed by the principal of the student's most recently attended school. 

• Please bring the required documents with you to your scheduled registration appointment. Failure to do so will either delay or prevent your child from starting Pre-K.
• Please allow approximately 1 hour for your appointment.

If another language other than English is the main language spoken at home, an additional English Language Learners Screening appointment will be required if accepted into the program.
