Proof of Residency

Parents and guardians may submit Proof of Residency documents at (1) the District Office, or (2) their student's school. 

For the 2025-2026 school year: residency documents can be uploaded in the registration portal for complete online registration. If needed, residency documentation can be provided at (1) the District Office, or (2) any school office.

Before any student is enrolled in ZESD 6, the student’s parent/guardian must prove their legal residence is within the District Boundary Map.

If you do not have a signed lease, if you do not rent, or if you do not own property, you must submit a notarized* Residency Affidavit Form.

If you do not have a lease, or are living month-to-month, please submit a notarized* Month-to-Month Rental Agreement Addendum.

*The District Office provides notary services at no charge; however, both the parent/guardian and the owner or lessee must be present for notarization.
