Health Needs by Grade
ALL students must have a medical/physical exam form on file according to the specifications noted below.
Proof of Pre-Kindergarten/Early Childhood Health Needs
- New Medical/Physical exam that includes updated shot records, including at least one dose of the Pneumococcal vaccine
- Lead screening
- Returning students need to have their physical on file with us and must have at least 1 dose of the Pneumococcal vaccine.
Kindergarten Health Needs
At Registration, all forms will be available if needed, in the Kindergarten Packet.
Dental Exam
Eye Exam
New Medical/Physical Exam with updated shot records, including:
- At least 4 DPT's with the last one on or after your child's 4th birthday
- At least 4 Polio with the last one on or after your child's 4th birthday
- 3 Hepatitis B
- 2 MMR
- 2 Varicella
- Lead risk questionnaire
- Diabetic screening
Grade 2 Health Needs
At Registration, all forms will be available, if needed.
Dental Exam
Grade 6 Health Needs
At Registration, all forms will be available, if needed.
Dental Exam
New Medical/Physical Exam with updated immunizations including:
- Tdap
- 2nd Varicella
- 1 dose of the Meningococcal vaccine (MCV) on or after your child's 10th birthday
- Diabetic screening
- Interscholastic sports marked YES if your child wants to do extra sports
Grade 7/8 Health Needs
At Registration, all forms will be available, if needed.
- Show proof of (1) dose of TDap
- Sports physicals must be renewed every calendar year.
New physical means a physical done within the year of the start date of school on the new form.
