THE DENTIST IS COMING TO SCHOOL! 

Get in-school dental care at NO COST for patients covered by Medicaid or All Kids!

Taking care of your child's teeth is important to keep them healthy. 

A state licensed dentist will check your child's mouth & teeth, as well as provide a cleaning, X-rays, fluoride treatment and apply sealants, as needed.

A dental report card will be sent home with your child. Permission includes initial dental care & follow-up visits.

Return all forms by September 26, 2025

How to participate:

  • Print and fill out the Asthma Van Consent Form, pick up a form in your student's school office, or complete the form online at: www.MySchoolDentist.com
  • Complete the Parent Section of the Certificate of Child Health Examination
  • Return ALL forms to the student's school health office by September 26, 2025

Eligibility details:

  • Free for patients covered by Medicaid or All Kids
  • or Pay $70.00 for a dental cleaning, screening, fluoride, and sealants

Appointments will be scheduled on a first come first serve basis.
