The Ronald McDonald Care Mobile is a doctor’s office on wheels delivering pediatric providers to your neighborhood. Now, children 4-18 years have more convenient access to health services.

Services include:
- Wellness visits
- Physicals for the 2025-2026 school year
- Immunizations
Additional Details:
- This program is for students who have Medicaid or No Insurance.
- Services cannot be provided for children covered by private insurance.
- All of their services are provided at no cost.
- Services are provided by Advocate Children’s Hospital.
Must sign consent form and return to school office by September 17, 2025.
The school nurse will schedule appointments from 9:00 A.M. to 2:30 P.M on a first-come, first-serve basis.

Is your child in need of a physical or immunizations? We’re offering free 30-minute appointments through our Lake County Asthma Van.

How to participate:
- Fill out the Asthma Van Consent Form
- Complete the Parent Section of the Certificate of Child Health Examination
Eligibility details:
- No insurance, or state insurance: you're eligible for free physicals and immunizations!
- Private insurance: students may only receive physical exams ONLY through the Asthma Van
Appointment options (30 minutes):
- Physical Only
- Immunizations Only
- Physical and Immunizations
Appointments will be scheduled on a first come first serve basis.
